POSITION OVERVIEW: The RN, Triage Nurse Hospice - Community Care works to provide quality and safe delivery of either Home Health, or Hospice care services to each departments respective patients during business hours. Works in collaboration with departmental patients primary nurses and clinical staff, Post-Acute Care Intake Services and Referral Services, and leaders to provide appropriate departmental services that reflect the Community Care Services philosophy and standards of care.
This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES:
Assesses the patients condition and needs using critical thinking and clinical judgment; sets outcomes; implements appropriate nursing actions and collaborates with other disciplines to meet the patients/familys physical, emotional, spiritual, social, and intellectual needs; and evaluates patients progress.
Responsible for assessing patients and determining their level of need for medical assistance and care.
Depending on the Community Care Department; this role will assign RNs, CNAs, Social Workers, and Chaplains (a care team) to each patient based on level of need.
Monitor calls, assignments, and check-ins.
Work with physicians and the appropriate Community Care team to update the plan of care, follow prescribed medical treatments, and provide education and supportive care to patients and their families.
Provides and maintains a safe environment for caregivers, patients and guests.
If needed, this role documents assessment, client/family response to nursing interventions, and appropriate home health care functions at the time of the visit.
Documents in Electronic Medical Record with proficiency.

Provides a therapeutic presence in service to others. Therapeutic presence is purposefully responding to the needs of patients/customers in a caring way. It includes introducing oneself and stating your purpose, asking the patient their preferred name, sitting with the patient to determine their care goals, active listening, and the skillful use of communication skills (touch, eye contact, etc.).
Responsible to manage the patients experience by clarifying, integrating, and coordinating the roles of the interdisciplinary team.
Functions as a member of the interdisciplinary care team to ensure effective case management of patient care, including optimum clinical outcomes.
Identifies patients appropriate for other appropriate services.
Orchestrates daily visits for caregivers in hospice.
Exercises clinical knowledge and judgment in assessing clients status and needs, and responds appropriately.
Consistently demonstrates technical and clinical competence that facilitates assessment of the clients status and needs.
Participates in staff meetings and in-services.
Meets departmental standards and deadlines for timely completion of documentation.
Develops and maintains knowledge of Federal and State Regulations, basic insurance knowledge, benefits, services, and limitations.
Conducts all activities with the highest standards of professionalism and confidentiality. Complies with all applicable laws, regulations, policies and procedures, supporting the organizations corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violation of applicable rules, and cooperating fully with all organizational investigations and proceedings.

EDUCATION
Required: As required by licensure.
Preferred: BSN.
LICENSURE/CERTIFICATION/REGISTRATION
Required: Current Oregon RN license.
Current American Heart Association Basic Life Support certification.
Valid Oregon driver's license and ability to meet SCHC driving requirements

Required: Three years experience as a staff nurse and one year experience in hospice care nursing.
